DUI Groups
All groups will be held via Zoom until further notice
Intake paperwork & an intake group are required to attend any DUI group or Substance Use Treatment , per OBH requirements
To start treatment with ASC please do the following:
1. Fill out the Intake paperwork via Docusign, click here for paperwork. (You must validate your email with DocuSign before you can complete the paperwork, please follow the instructions provided by DocuSign). If you are attending for a charge that happened outside of the state of Colorado, STOP and call call 970-945-7858
2. Once paperwork completed, you will receive an email (within one to two business days) with information regarding your counselor & intake time
3. You must attend this intake group before you start treatment, your counselor will help assign you to a group
4. Please note, there is one book fee for DUI Education books and another book fee if you are attending Therapy
5. Please use the button below to upload Court Orders, DRS from another agency, etc, so we may better assist you
